Position Overview & Specifications
Supporting the development and delivery of an Integrated Neighbourhood in North Care Partnership, which will have a Primary Care Network (PCN) at its core..Your role will be to bring partners and communities together, helping Integrated neighbourhoods to identify how they can improve care and support, and deliver this change and transformation.
You will act as a relationship builder and connector helping to join partners up and try new ways of working at a time of significant pressures and challenges. Providing transformation and project management expertise to make positive changes, build strong and sustainable foundations/relationships to tackle inequalities.
Working closely with the GP Clinical Lead and clinical teams to review their population health data and supporting public engagement to determine how the local population want their health and care services to improve. You will be responsible for supporting the service redesign and delivery of specific projects within the Integrated Neighbourhood, and delivery of the integrated working improvement projects which have been developed by North Care Partnership.
Key Responsibilities
- Work closely to build relationships and bring partners and communities together to make positive change
- Work closely in partnership with clinical leaders, the acute sector, community services, health commissioners, councils, voluntary and community organisations, and the public to support the development of the service design
- Facilitating partnerships across all partners and supporting cross organisational and cross boundary working
- Project lead for specified Integrated Neighbourhood(s) within one of the Locality areas in the North Care Partnership, to enable delivery of specific priorities
- Project support to cross-organisational meetings including co-production and presentation of highly complex and sometimes contentious information to support decision making, and design of agendas to enable key decision making.
- Support, facilitation and delivery of the Integrated Neighbourhood.
- Support, facilitation and delivery of integrated working improvement projects.
- Assurance that specified programmes of work are being delivered within budget and in accordance with the overall plan.

In addition to supporting a specific Integrated Neighbourhood within a locality the post holder will support the delivery of the integrated working improvement projects which have been developed by the North Care Partnership. This programme of work aims to make operational changes to improve cross-organisation working and remove the barriers to integration.
This role requires close partnership working with clinical leaders, the acute sector, community services, managers from the ICB, local authority, voluntary organisations, and patients to support the development of the service design. It requires the ability to support the translation of a clinical vision into a coherent operational model and project delivery.
