Home First model relates to patients discharged through pathway 1 of the national Hospital Discharge and Community Support: policy and operating model i.e. those who are able to return home with new, additional or a restarted package of care from health and/or social care. Every effort should be made to follow Home First principles, allowing people to recover, reenable, rehabilitate or die in their own home. Home First pathway is a 5 days Monday to Friday 09:00 to 17:00 service Communicate and engage with Acute Partners, CCG, Local Authorities, and other Integrated Discharge Hubs to ensure the discharge to assess pathway is seamless
To assess the Physiotherapy needs of a defined patient group and establish and evaluate appropriate treatment plans. This includes chronic, acute, and palliative care within own competencies, recognising own limitations and seeking advice when necessary.

The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will supervise pre-registration students. They will have a commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies.

High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,108 and a maximum payment of £5,177).

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction.
