Position Overview & Specifications

Stroke and Neuro Pathway Facilitator role- therapies. Band 4 30- 37.5 hours

The Stroke Pathway Facilitator will act as the point of contact for the Multidisciplinary Team (MDT) staff, facilitating an effective and smooth discharge from therapy- this will include carrying out home visits, ordering of equipment, completing of referrals, completing follow up phone calls or visits as required at the request of a registered therapist.

The Stroke Pathway Facilitator may hold a small, designated caseload for which they can carry out basic assessments, conduct a rehabilitation programme and plan discharge (with guidance as required from a registered therapist).

Along with the patient key worker, they will ensure that therapy aspects of the patient’s discharge is progressed and escalate when there are changes or delays to the Estimated Date of Discharge (EDD), patient needs or clinical condition.

A key part of the role will be to ensure there is early communication with families/carers/care homes etc, to ensure all relevant information is collated and potential barriers are highlighted so they can be addressed and solved prior to when the patient leaves hospital and mitigate failed discharge.

The Stroke Pathway Facilitator will work holistically across rehabilitation disciplines and may, where appropriate see patients in an ambulatory care, or community setting.
  • Autonomous provision and progression of ongoing therapy (across Physiotherapy, Occupational Therapy and Speech and Language Therapy) in accordance with therapy treatment plans to stroke and neurology outliers (those not on the stroke unit)
  • To work as part of the stroke and neuro therapy team based at Dorset County Hospital (stroke unit and other outlying wards).
  • Undertaking a range of basic assessments (in line with band 4 competencies and framework)- this may include assessments patients home environment
The Dorset County Hospital Therapy Team is looking for an enthusiastic, reliable team player to join the Stroke and Neuro team as a Stroke Therapy Pathway Facilitator (Band 4).

Band 4 experience in healthcare essential, as well as stroke and neuro experience (in any setting) as a therapy assistant. Basic IT and administrative skills required. Car drivers only as part of role will be supporting home visits. Relevant courses at Foundation Degree level or equivalent are expected to be completed (can be completed whilst in role).

The successful applicants will assist in treating a wide variety of clinical conditions and work alongside both Occupational Therapy, Physiotherapy and Speech and Language Therapy, delivering rehabilitation and assisting in the safe and timely discharge of patients. We provide supervision within the team and have training provided for therapy assistants. You are encouraged to develop through appraisals and CPD.

The Therapies service at Dorset County Hospital is an integrated Therapies Service working together to ensure that Therapies provision is the best that it can be, balancing quality and resourcing issues to provide and improve patient centred services. The Therapies team is committed to the principle of seven day working and is currently working towards it. Dorset County Hospital NHS Foundation Trust is a modern District General Hospital with around 500 beds, offering an excellent working environment. The area is largely rural, with good rail and road links. The closest airport is situated in Bournemouth.

  • Undertaking a range of basic assessments (in line with band 4 competencies and national recommendations)- this may include assessments patients home environment
  • Training and support of band 3 assistant staff and supporting rotational band 5's
