Position Overview & Specifications

Health and Wellbeing Worker Band 5

The post holder will be responsible for the provision of follow up care to patients referred to Croydon health Service (CHS), Drug & Alcohol Support Service (DASS) in and outside of the hospital setting.

The post holder will alongside their DASS colleagues develop a procedure and pathway to cater for those patients attending Croydon University Hospital (CUH) Emergency Department outside business hours that would otherwise not be followed up.

The post holder will contribute to the quality and development of holistic nursing care provided to patients in CHS by DASS.

The post holder will work with DASS develop pathways in and out of CUH for those patients that are difficult to engage.

The post holder will liaise with appropriate internal departments and external agencies to enhance the care provided for DASS patients.

The post holder will work across the organisation to support the assessment and management of patients presenting with addiction

Croydon Health Services have a developed a service for people attending the Croydon Health Services who are living with addiction to alcohol or other substances.

The Drug and Alcohol Support Service (DASS) is an innovative service that is designed to improve the experience and health outcomes for people using Croydon Health Services and experiencing ill health as the result of addiction or are experiencing addiction as a result of ill health. The team also offer plans of care for people admitted to the hospital with acute physical health needs and addiction.

We are looking for a band 5 health and well being worker who will work closely with our Emergency Department colleagues to support people who have attended the department out of the DASS hours. We are also looking for someone who can work within CHS services, offering assessment and ongoing care planning that supports people towards recovery. The post holder will support people to attend community locations for initial assessment and work to engage people to support them moving into management and recovery. The post holder will work closely with colleagues within he commissioned community addiction service as well as colleagues across Croydon Health Services.

The post holder will be expected to begin engagement work with patients who admitted to the hospital We welcome applications from people with lived experience of addiction and recovery.

The Trust was formed on 1st August 2010 through the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust. We’re also leading the way in providing more healthcare outside hospitals at clinics, specialist centres and in people’s homes.
