Position Overview & Specifications

The Community Independence Service (CIS) Urgent Community Response (Rapid Response - Physical Health) is a service that aims to bring the best of hospital care to a patient’s home avoiding unnecessary hospital attendances/admissions.

We are looking for experienced nurse/paramedic with a passion for patient care and drive, enthusiasm and talent to lead a team. You will want to develop your skills and push yourself outside the normal boundaries of practice and will have a real drive to make sure your patient gets the best treatment possible. Excellent communication skills, a strong work ethic and the ability to lead an experienced group of clinicians are essential and in return we offer significant development opportunities.

Our referrals come from all avenues of the wider system including GPs/NHS 111/ London Ambulance Service.

We provide a Urgent Community Response service that brings an initial holistic assessment to the patient’s home within two hours, identifying issues and developing assessment and treatment plans to come up with appropriate differential diagnoses which may include doing a range of diagnostic tests.

Come and join our team and give care to those in the community settings across inner London – based in Westminster/K&C or Hammersmith and Fulham.

As a Band 7 Urgent Community Response Practitioner you will:
  • Provide the highest level of clinical and managerial leadership to the team on shift and support the development of the Service through leading on projects and audits.
  • Be the Clinical Coordinator of the team on shift, triaging calls, offering excellent customer service and ensuring that the most appropriate clinician or service for the patient's needs is provided. You will also take handovers help devise treatment plans and manage service capacity
  • Be the first point of clinical advice and guidance for clinicians in the field caring for patients.
  • Be involved in building relationships with sister-services and stakeholders
  • Attend patients to do an Urgent Community Response initial assessment and for follow up visits.
  • Develop your clinical skills into a more well-rounded holistic approach to patient care.
  • Develop new skills outside of your normal scope of practice including skills from Paramedicine, Nursing, Physiotherapy and Occupational therapy.
  • Supervise team members and students
  • Attend in-service training and have Continued Professional Development opportunities
  • CIS Urgent Community Response works 7 days a week from 8:00am until 22:00pm. You will be expected to work weekends and bank holidays although these will be distributed throughout the team.
  • Shift patterns are made up of 12 hour days.
