Position Overview & Specifications
As a Care Coordinator at Carer Connect Limited, you will be responsible for coordinating and scheduling care services for clients in their own homes. You will work closely with the Care Manager, clients and their families, and a team of care assistants to ensure that clients receive the best possible care and support to help them maintain their independence and quality of life.
Responsibilities:
- Work with the Care Manager to develop and implement care plans for clients
- Coordinate and schedule care services for clients in their own homes
- Assign tasks and delegate responsibilities to care assistants
- Ensure compliance with all policies and procedures related to client care and safety
- Communicate regularly with clients and their families to assess their needs and preferences
- Monitor and report on client progress to the Care Manager
- Participate in the recruitment and training of new care assistants
- Maintain accurate records of client care and progress
- Attend training and development sessions to enhance your skills and knowledge
- Maintain a professional and caring demeanor at all times
Requirements:
- Minimum of 1 year of experience in a care coordination role, preferably in domiciliary care
- Knowledge of health and safety regulations related to domiciliary care
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Must have a valid driver’s license and access to reliable transportation
- Must be able to work flexible hours, including evenings and weekends
- Must be able to pass a background check
Mandatory:
- Uk Clean Driving Licence (Full)
- Minimum 2 Years of Visa Status with Right-To-Work

Salary: £19,000.00-£20,000.00 per year

Experience:
- Care Coordinator: 1 year (required)
