Position Overview & Specifications

As a Reablement Worker with Brunelcare you will be part of the team in the short stay reablement centre, focusing on restoring a person’s independent function following an emergency hospital admission.

About the role

  • To holistically support individuals after a period in hospital with a reablement plan and work with them to find ways to meet their desired outcomes with a view to regaining as much independence as possible.

  • Liaising with health care professionals in order to review the progress of people utilizing the service and facilitate discharge home, where possible.

  • Participate and assist with any needs assessments, produce relevant Brunelcare documentation and complete weekly reviews of outcomes.


About you

  • Ability to work under your own initiative and be part of a close knit team.

  • Possess and demonstrate a competent and professional manner regardless of the situation.

  • A commitment to enabling people to live their lives as well as they can.

  • Previous healthcare experience would be an advantage.
