Position Overview & Specifications
- To complete exercise testing with patients to monitor improvements in their fitness levels.
- To develop innovative ideas for encouraging patients to participate in rehabilitation programs and overcome their barriers to exercise.
- improve and promote staff knowledge surrounding health and well being and how to embed conversations about weight and physical activity into routine care.
- To uplift Trust staff knowledge and contribution to raising awareness of, and knowledge of physical activity, healthy weight and health diet, referrals to weight management and signposting to activity advice/services
- To support with managing and maintenance of accurate databases and data collection for patients within the service.
- To support with the Alder Active Programme with wider Health Promotion and supervising sessions.
- To link with community provisions and external partnerships (for example Liverpool Football Club Foundation) and facilitate the transition of the child and young adult into community programs.
- To support children and young adults engaging with the CEW service and monitor engagement over an extended period including follow up appointments and review.
- To liaise with and report to Specialist Physiotherapist and attend regular supervisions.
